The following information will be needed for a home oxygen discharge:

Dr.’s Prescription with the following information needed prior to delivery:
Patient’s Name and Address
Diagnosis (specific to equipment)
Liter Flow and Hours Needed (E.g. O2 at 2 liters Continuous)
O2 Saturation (88% or below for insurance billing)
Ambulatory Status
Dr.’s Name, Address
Dr.’s Signature
Date

A copy of nursing notes showing, O2 Saturation level of 88% or below taken within 2 days of discharge.
